Class II Licensing Requirments and Classifications

The Puyallup Tribe of Indians has three classifications for a Class II Employee:

Class II-A Primary Management:

Primary management are generally considered to be up to the top 5 management personnel in a facility, i.e. General manager, Controller etc.

Class II-A Key Personnel

Key Personnel means all employees who have authority over receipt or distribution of revenues from the gaming activity, and shall include but not be limited to cashiers, dealers, pit bosses, floor managers, supervisors and any other individual who has the authority to sign checks or handle cash or currency. The Puyallup Tribal Gaming Regulatory Office also includes those individuals with access to key information or areas of the gaming establishment.

Any individual offered employment at the gaming establishment is told what type of license they need to obtain and what the appropriate fees are.

A Class II-A license requires that only the Tribal Gaming Agency conduct a background to include verification of employment for the last 10 years, credit, and criminal records for life. This license requires that the Puyallup Tribal Gaming Regulatory Office issue a tribal license upon recommendation from the Licensing Department.

Class II-B:

Generally a Class II-B licensee is working in facilities, or food and beverage.

A Class II-B license requires that only the Tribal Gaming Agency conduct a background to include verification of employment for the last 10 years, credit, and criminal records for life. This license requires that the Puyallup Tribal Gaming Regulatory Office issue a tribal license.

Class II New Certification

Required Forms

Class II New Certification for employees of the Puyallup Tribes Bingo Palace and BJ's Enterprise

    • Completed Puyallup Tribal Gaming Regulatory Office Personal/Criminal History statement
      • All questions must be answered completely, number of years lived in Washington, Drivers license number (if no license then put in State ID and Number) physicals etc., if not applicable put N/A in box.
      • Employment section includes any periods of Employment, Schooling, or unemployment. Use month and year format only, every month for the previous 10 years must be accounted for.
      • The same format as employment is to be applied to residence history.
      • Business licenses ever held, currently applied for, or have been denied, revoked or suspended in any other state must be completed.
      • Criminal History must be completed for entire life. This section requires any traffic for which you paid a fine of $25.00 or more. Any Criminal history as an adult or juvenile must be disclosed on this form. Included must be the actual charge, the City, County and State in which the arrest, charge or conviction took place. Disposition must be included, if this is a recent charge, a felony or if the individual claims paying on the fine then it is a good idea to inform them to get copies of any pertinent court documents or proof of payment arrangements from the court and have them ready if requested from the Tribal Gaming.
    • Completed Class II Addendum
      • The addendum is basically the same information as the State Application with the following exceptions or changes;
        • Residence section only requires City, County, State and month and year.
        • List all languages spoken or written on page 2.
        • Criminal History requires disclosure of any Felony ever. If the applicant has a felony record or pending felony charge they must disclose the exact charge, date occurred, city in which occurred, name and address of courts involved and final disposition. If pending the next court date and what it is for, i.e. pre-trial, evidence hearing, etc., etc.
        • Employment only requires five years and should correspond with other forms, on the addendum Phone Number is required for the employer.
        • Any business the applicant has owned or held interest in for the last 10 years.
        • The applicant must provide three references with complete street addresses that have known the applicant for at least three years.
        • The applicant must answer the questions on page 4 of the addendum and explain any yes answers.
        • Any previous licenses granted for the purpose for a gaming requires disclosure of type of license, agency issuing license and address of agency issuing license. Note Tribal Gaming Regulatory Office issue Class III Indian Gaming Employee Licenses, Washington State Gaming Commission issues certifications.
        • Any professional or privileged licenses with any regulatory agency would include business licenses, conceal weapons permits, security guard licenses, beautician licenses, Washington State Gaming Commission certification, etc. etc.
        • The applicant is must disclose High School attended, Colleges etc., complete address for schools, degree received, and year graduated.
        • Other training and certificates would include dealer schools, bartending academy, etc.
        • The last page requires the applicant read and signs the Certification/Authorization/Release in the presence of a notary.
  • Other Documents required include:
  • One Photo taken within the last 30 days, Name, social security number and date of photo included on the photo.

    Two copies of Identification. Acceptable forms of Idea standardized to the same requirements as the Immigration and Naturalization form I-9.

    If a resident alien, completed resident alien information form with copy of front and back of I-551 card, and copy of foreign passport.

    If a Naturalized citizen, a copy of certificate of naturalization.
