The Puyallup Tribe of Indians has two classifications for a Class III Employee:
Class III-A Key personnel :
Key Personnel means all employees who have authority over receipt or distribution of revenues from the gaming activity, and shall include but not be limited to cashiers, dealers, pit bosses, floor managers, supervisors and any other individual who has the authority to sign checks or handle cash or currency. The Puyallup Tribal Gaming Regulatory Office also includes those individuals with access to key information or areas of the gaming establishment.
The State of Washington defines a Key Employee as any individual employed in the operation or management of Class III Gaming in connection with the Tribe's gaming operation or facility, whether employed by or contracted to the Tribe or by or to any person or enterprise providing gaming operation or management services to the Tribe. Including but not limited to, gaming operation managers and assistant managers, accounting personnel, surveillance and security personnel, unless employed by the Tribal Gaming Regulatory Office, cashiers, supervisors, dealers, box men, floormen, pit bosses, shift bosses, cage personnel, collection personnel, gaming consultants, parimutuel clerks. management companies and their principals, and any person whose employment duties require or authorize access to areas of the gaming facility related to gaming which are not otherwise open to the public, or to areas designated by the Tribal and State Gaming Agencies.
Any individual offered employment at the gaming establishment is told what type of license they need to obtain and what the appropriate fees are. A Class III-A license requires that not only the Tribal Gaming Agency conduct a background to include verification of employment for the last 10 years, credit, and criminal records for life, but also that the Washington State Gambling Commission conduct a background investigation. This license requires that not only the State issue a Class III Indian Gaming Certification but also that the Puyallup Tribal Gaming Commission issue a tribal license. An applicant may qualify for one and not the other.
Class III-A New Certification
Class III-A New Certification for employees of the Puyallup Tribes Emerald Queen Casino
1. Washington State Gambling Commission Class III Indian Gaming Management/Employee Certification Application:
Completed form including any previous gaming license for any jurisdiction.
Complete employment section listing Puyallup Tribe of Indians, as Name of Tribe and Name of Employer should read Puyallup Tribe's Emerald Queen Casino. Employment date must be left blank on this form.
Oath of applicant and Waiver for Tribal Members and Native Americans signed if applicable.
Tribal authorization must be left blank. If the applicant signs in this box they must do a new form.
2. Completed Washington State Gambling Commission Personal/Criminal History statement
All questions must be answered completely, number of years lived in Washington, Drivers license number (if no license then put in State ID and Number) physicals etc., if not applicable put N/A in box.
Employment section includes any periods of Employment, Schooling, or unemployment. Use month and year format only, every month for the previous 10 years must be accounted for.
The same format as employment is to be applied to residence history.
Business licenses ever held, currently applied for, or have been denied, revoked or suspended in any other state must be completed.
Criminal History must be completed for entire life. This section requires any traffic for which you paid a fine of $25.00 or more. Any Criminal history as an adult or juvenile must be disclosed on this form. Included must be the actual charge, the City, County and State in which the arrest, charge or conviction took place. Disposition must be included, if this is a recent charge, a felony or if the individual claims paying on the fine then it is a good idea to inform them to get copies of any pertinent court documents or proof of payment arrangements from the court and have them ready if requested from the Tribal Gaming.
The applicant must read and sign the certification at the bottom. Place is city in which the application was signed.

Other Documents required include:
Two Photos taken within the last 30 days, Name, social security number and date of photo included on the photo.
Two copies of Identification. Acceptable forms of Idea standardized to the same requirements as the Immigration and Naturalization form I-9.
If a resident alien, completed resident alien information form with copy of front and back of I-551 card, and copy of foreign passport.
If a Naturalized citizen, a copy of certificate of naturalization
Class III-B Non-Key Personnel:
Generally a Class III-B licensee is working in facilities, or food and beverage.
A Class III-B license requires that only the Tribal Gaming Agency conduct a background to include verification of employment for the last 10 years, credit, and criminal records for life. This license requires that the Puyallup Tribal Gaming Commission issue a tribal license. An applicant who does not qualify for a Class III-A license may qualify for a Class III-B.
