Vendor License Fees

The following fees are in effect as of February 1999:

Vendor Principle Company Application Fee $1000.00

Vendor Employee Fee $250.00

Vendor Company Renewal $500.00

Vendor Employee Renewal $150.00

Use of Application Fee-Additional Fees Required.

The application fee must be used to conduct the background investigation of the applicant and to defray administrative expense. If the Executive Director determines that additional sums are needed to continue or complete the investigation of an applicant, the processing of the application must cease, and the Executive Director must inform the applicant of the additional sums required. As soon as the applicant has furnished the additional sums the processing will continue.

Any vendor wishing to receive a license with the Puyallup Tribal Gaming Regulatory Office must submit the necessary forms and initial fee to begin the application process. During the process the Gaming Regulatory Office must conduct an on site inspection of the vendors business premises. The fee for the on site inspection is billed to the vendor.

Vendor Licensing Instructions

The following are instructions for applying for a Vendor License with the Puyallup Tribal Gaming Regulatory Office.

An application for a Puyallup Gaming Vendors License will need to be submitted with supporting documentation for the business proposal outlining:

a. Organizational Structure (Articles of Incorporation if incorporated)
b. Identification of all Principals
c. Stockholders over 10% must complete 10 year personal information
d. Copies of all Contracts pursuant to all business conducted with the Puyallup
Tribe Gaming Activities
e. Any other documents requested by the Puyallup Tribal Gaming Regulatory Office or Licensing Department deemed to be necessary for completion of a background investigation
f. Copies of all State Certification and Licensing Certification including Gambling Certification.
g. Completed Gaming License applications for all employees that will be potentially on site at a Puyallup Tribal Gaming Establishment, all principals and all shareholders with a 10% or more interest in the business.
h. Master business application with source of funds and financials.
i. Appropriate fees for license.

Application Instructions:

1. All applicants and substantial interest holders will be required to provide positive proof of their identities.
2. Please type or print in Black Ink.
3. Answer all questions. Use N/A if not applicable. If necessary, you may copy any of the forms in the application packet for submission.
4. Mail or deliver the completed application, fee(s), and all accompanying documents to the above address.
5. Ensure that the application is signed and dated by the appropriate individual(s). The application requires an original signature.
6. When completed, the application and addendum should be rechecked to ensure completion. This check may help to avoid delays during the processing of the application. Incomplete applications will not be processed.

* AS PART OF THIS APPLICATION, YOU ARE REQUIRED TO PROVIDE POSITIVE PROOF OF IDENTITY. TO ACCOMPLISH THIS REQUIREMENT SUBMIT, ALONG WITH THIS APPLICATION, THE FOLLOWING ITEMS. A COPY OF ONE OF THESE OFFICIAL DOCUMENTS: A BIRTH CERTIFICATE, A VALID DRIVERS LICENSE, A MILITARY IDENTIFICATION CARD, A VALID PASSPORT, OR IF A REGISTERED ALIEN, AN ALIEN REGISTRATION CARD. ALSO SUBMIT A COPY OF SOCIAL SECURITY CARD AND A CURRENT FULL FACIAL VIEW PHOTOGRAPH NO SMALLER THAN 1" X 1" NOR LARGER THAN 3" X 5". WRITE YOUR NAME AND SOCIAL SECURITY NUMBER ON THE BACK OF THE PICTURE. YOU WILL ALSO BE REQUIRED TO SUBMIT FINGERPRINTS, THESE MUST BE DONE BY A LAW ENFORCEMENT AGENCY AND THAT AGENCY MUST FILL OUT THE EMPLOYER INFORMATION ON THE CARD, see sample sheet enclosed in application packet.

ENSURING THAT ALL PERSONAL AND CRIMINAL HISTORY FORMS ARE COMPLETED IN FULL AND SUBMITTED WITH THE WAIVERS AND RELEASE FORMS WITH THIS APPLICATION; FOR EACH INDIVIDUAL WORKING WITH THE PUYALLUP TRIBE AND THE PUYALLUP TRIBAL GAMING Regulatory Office.
